NOTE: These form links will take you

- Scholarship-only Application - By completing the
scholarship-only application, you will be awarded only institutional
aid that you are eligible for. You must be enrolled full-time
to receive institutional funding. Only complete this form if
you choose not to complete a FAFSA.

- Legacy Grant - The Legacy Grant form should be filled out only by students who have a Parent, Grandparent or Sibling who has GRADUATED from Central Methodist University.

- Itemized Worksheet - This is to be completed by
students who have unusually low income reported on the FAFSA.
This will clarify how you and/or your family were supported during
the year at verification.

- Release of Financial Information - This form will give
the office of Financial Assistance permission to release information
pertaining to the students financial aid to the persons designated
by the student.

Consortium Agreement - This agreement is written between two
title IV eligible schools allowing a student to receive federal student
aid for study at a school other than his/her home school.
