Athletic Paperwork - Homelink Access

Any student who is going to participate in intercollegiate athletics at Central Methodist University is required to provide the Athletic Training/Sports Medicine Department with the appropriate medical information in order for the student athlete to be eligible to practice and compete. If you need to purchase Primary Health Insurance please visit www.maksin.com/CMU.shtml.

All athletes must submit information to Central Methodist University via the internet and also send hard copies of all medical forms to the Athletic Training/Sports Medicine Department prior to participation. It is the student athlete’s responsibility to keep the Athletic Training/Sports Medicine Department informed of changes in personal information such as addresses, emergency contacts, phone numbers, and any insurance policy changes. Using the Homelink system will make this process more efficient for the student and staff.

Instructions:

1. Greeting Letter and Medical Policy ( Please read first)
   


2. Information on your User Id and Password for Homelink.
   


3. Access Athletic Paperwork Homelink to submit all personal and health questionnaire information. Printable directions for Homelink page.
   Note: Freshman and Transfers will fill out all information on Homelink. Returners will only have to review, make changes and fill out missing information under Athlete Information. Everyone will fill out a new Medical History form.
   


4. Please print, sign and send the following form as well as a photo copy (front and back) of the student’s insurance card as proof of insurance.
   
• Acceptance of Risk and Liability Waiver


5. Print and submit all forms (Homelink Forms, Waiver forms and copy of insurance card) and mail signed hard copies to:
